Source of the Leak
Exophase began automatically adding hundreds of new Steam entries in a short period. Mike Bendel, the owner of Exophase known online as x3sphere, addressed the situation. He stated the leak appeared to be a mistake on Valve's end, where achievements meant to remain private became visible through the API. The site's scanner then picked them up. Valve has not made an official statement regarding the incident.

The leaked data covers both officially confirmed titles and a large batch of previously unrevealed projects. Major announced games with newly exposed achievement lists include Persona 6, Kingdom Hearts 4, Gears of War: E-Day, and Tomb Raider: Legacy of Atlantis.

For Kingdom Hearts 4, the leaked achievements reportedly reference Disney worlds that have not been officially confirmed. Players wishing to avoid spoilers may need to steer clear of achievement-tracking sites.
Caveats and Unconfirmed Projects
It is critical to note that an achievement listing on Steam's backend does not confirm a game's existence or eventual release. Some entries could be internal tests, placeholders, or cancelled projects. Publishers have not confirmed any of the unannounced titles.
One peculiar entry is for an unannounced Twin Peaks game. It currently lists only two achievements: a placeholder test and one for reaching the Double R Diner from the show. There is no confirmation of a developer or if it is an active project, and it may be an abandoned prototype.
